Sometimes people need to cancel their appointment which will open up the appointment again.
You will receive a confirmation e-mail or text.
You will need your confirmation or number code from the NYSDOH online attestation with you on clinic day and/or if you have to cancel due to illness, have had close contact with a person who has tested positive or a schedule change.
If you have a printer, print out your NYSDOH documents.
You will complete demographic and attestation forms at the Pop-up location or the Genesee or Orleans Health Department clinic location.
Follow the instructions on your confirmation notice and wait for a Health Department employee to assist you.
You will complete forms at the clinic site. Please bring a pen with you.
Show up for your scheduled appointment as long as you:
When coming to any of our clinics you are to wear face coverings per CDC/NYS guidelines.
Do not have any COVID-19 symptoms, flu symptoms or cold symptoms,
Or you have not been in close contact with someone who has tested positive for COVID,
Or you have recently recovered from COVID-19.
Make sure you are wearing a loose sleeve shirt as the vaccine will be in the upper arm.
For First (1st) Dose:
Please make sure you keep that appointment.
Bring your confirmation e-mail/text with you.
You will have to go into the building for the vaccine.
